According to Wisconsin ACT 172, all parents and players must read, complete and sign the concussion waiver forms. (T-ball and Rubber Ball age kids only require a parent signature) Forms should be turned in at registration or to your coach by the 1st practice. Your child will not be allowed to participate until the Concussion form is turned in.

The form below should be completed if you are applying for Umpiring, Concessions, or Field Maintenance duties.
